One week before your committee meeting, please do the following:
The form will be used to generate an automatic email to the committee members, reminding them of the meeting, time and location, and providing them with a link for their form to fill out.
It is important to note, we will not send them all the information you provide, as some of it must be kept confidential. We will only send them your name, meeting date, time and location.
Some things to note:
Your committee will fill out their form on their device during the committee meeting and submit it at the end of the meeting. You and your committee will receive an email with contents of the completed form.
