Fillable Forms
Can you save a fillable form?
Unless the user has Adobe 9 Professional, fillable forms cannot be saved. A fillable form must be printed once it is completed if the user wishes to keep a copy of the form.

Academic Assessment Report
How does the student submit their Academic Assessment Report to their referees due to the fact that the Academic Assessment Report is a fillable form?
If the student has Adobe 9 Professional, he or she can fill out Section A of the report, save it and then email it to their referee to complete and sign it. If the student does not have Adobe 9 Professional, the student must complete Section A and then print it for their referee to complete Section B manually and sign it.

NOTE: If a referee is giving the completed form to the student, it must be in a signed sealed envelope which the student is to hand in, unopened to the department.

The following information may also be helpful regarding the saving function for fillable forms:
After completing Section A of the Academic Assessment Forms, students can save the form using a free software package titled, "Foxit Reader”. It can be accessed and downloaded from the following link: http://www.foxitsoftware.com/pdf/reader
Also note that if the student scans the document and sends it to the referee via email, the student must ensure that the report is clear and legible.

Social Insurance Number (SIN)
On the OGS application form, candidates are asked to provide their SIN. Since international students are eligible for this scholarship but do not have a SIN number, can they leave this field blank?
Students who do not have a SIN must apply using the OGS paper application form as the online form will not let them continue without it. Therefore when international students complete their paper application, they can leave the SIN field blank.

Supporting Documentation
Where do the Summary Sheets and other supporting documentation get submitted?
All students are to submit the original and 5 copies of their application as follows:

  • Downtown: Daphne Sears at Princess Margaret Hospital, Room 7-411.
  • Sunnybrook: Merle Casci, Sunnybrook, Room S670.

The deadline for applications to be submitted to the MBP Office is Friday, October 8, 2010.

Technical Issues
Who do students contact when they are experiencing technical difficulties with the OGS online application and related OGS forms?
Students who are experiencing difficulties with the OGS online application and the related OGS forms are asked to send an email to the OSAP Webmaster detailing the problem. The OSAP Webmaster email address is: webmaster@osap.gov.on.ca.
VISA students
Can VISA students apply for an OGS award?
Students with a citizenship status of “Student Visa” may be considered for OGS awards that are categorized as Visa Awards only. “Student Visa” is defined as a temporary resident visa as a member of the student class under the Immigration and Refugee Protection Act (Canada). The maximum number of Visa Awards for any academic year is 90. Students in this category must meet all OGS eligibility criteria. Students with a citizenship status of “Student Visa” cannot be nominated for institutional awards.

Change Form
When do I need to use the Change Form?

The Change Form is only to be used if there are changes to the following items of the OGS application form:  student number, name, address, phone number, date of birth, citizenship status, discipline code and level of study for Proposed and Current Studies and number of OGS awards previously received. The change form is not to be used for anything else.
